Depression Care Management Programs
Should include:
- Education regarding depression and its treatment.
- 6 monthly outreach calls in the first 6 months.
- 6 quarterly outreach calls between 7-24 months even if the patient feels better.
- Depression care manager contact with the healthcare professional if the patient falls to improve or relapses. (with patient permission)
