Depression Care Management Programs

Determine whether you have a Depression Care Management benefit. Many employers contract with health plans, disease management vendors or employee assistance programs to provide this service free of charge to their employees. If your employer does not provide you a Depression Care Management benefit, you can purchase it individually.

  • The nurse in your doctor’s office can contact your employer to see if you have this benefit without revealing your name.
  • You or the nurse can contact one of the the vendors listed in our Depression Care Management Vendors Chart to determine if the vendor provides Depression Care Management to employees in your company.
  • The vendors who provide this service to individuals who want to pay directly for it can also be found in our Depression Care Management Vendors Chart.

Whether you get the benefit free of charge or pay for it out of pocket, please make sure your Depression Care Management program includes:

  • Education regarding depression and its treatment.
  • 6 monthly outreach calls in the first 6 months.
  • 6 quarterly outreach calls between 7-24 months even if you feel better.
  • Care manager contacting your doctor with your permission if you fail to improve or begin to feel worse.

Depression can take away your best moments... but it can be successfully treated.
